NOTE: Recreational activities, including aerial manoeuvres (somersaults, flips, twists) involve an inherent risk of significant physical harm, including personal injury, permanent disability and/or death. Such harm may result from your actions, or the actions and/or omissions of others. I, the below named person, in participating in any Queensland Diving Association Inc (DQ / Diving Queensland) recreational events (Activities) in the sport of springboard and platform diving organised by DQ hereby acknowledge, understand and agree to the following: 

1. Fitness to Participate: I declare that I am medically and physically fit and able to participate in the Q Splash Championships, associated training events, and spectator "Splash Zone". Q Splash will include activities in the Dryland, the pool and other general fitness and conditioning activities (collectively known as “Diving Activities”). I will immediately notify Diving Queensland of any change to my medical condition, fitness or ability to participate. I understand and accept that Diving Queensland will continue to rely upon this declaration as evidence of my fitness and ability to participate. I agree to advise DQ in writing of any disability or medical problem which may cause or promote any injury or illness.
2. Inherent Risks. There are inherent risks associated with diving and the use of dryland and pool facilities (including, but not limited to diving boards, trampolines, gymnastic equipment, swimming pools, and foam pits), which may result in personal injury (even of a serious nature) to participants. I fully accept and agree to bear these risks.
3. To the full extent permitted by the Competition and Consumer Law Act 2010 (CCA), the Australian Consumer Law, Queensland Civil Liability Act 2003, the Personal Injuries Proceedings Act 2002, and Common Law, I absolve, release, discharge and indemnify Diving Queensland, Stadiums Queensland, Gold Coast City Council, Brisbane City Council, and its/their officers, employees, servants, representatives and agents (Indemnities) from any and all liability for any personal injury, mental anguish, loss, damage or expenses of any kind suffered by me, however caused arising out of my participation in the Recreational Diving Programme, including without limitation, where caused by any acts of negligence by the Indemnities.
4. Medical Treatment: I consent to receiving any medical treatment that Diving Queensland, Gold Coast Aquatic Centre, venue lifeguards, or venue staff reasonably considers necessary or desirable for me during my participation in Diving Activities. I also agree to reimburse the relevant Organisation for any costs or expenses incurred in providing me with medical treatment.
5. Right to Use Image: I acknowledge and consent to photographs and videos being taken of me during my participation in any Diving Activities. I acknowledge and agree that such photographs and videos are owned by Diving Queensland and/or external media outlets and may be used for promotional or other purposes without my further consent being necessary. Including use by Quantum Events as part of promotional activities relating to the Z Manu World Championships. Further, I consent to Diving Queensland using my name, image, likeness and also my performance in the Diving Activities, at any time, by any form of media, to promote the Diving Activities.
6. Privacy: I understand that the information I have provided is necessary for the objects of Diving Queensland. I acknowledge and agree that the information will only be used for the objects of Diving Queensland. I understand that I will be able to access the information through Diving Queensland. I acknowledge that the Diving Queensland may also use my personal information for the purposes of providing me with important event updates, including promotional material from Diving Queensland sponsors or third parties. I may advise Diving Queensland if I do not wish to receive from Diving Queensland, any sponsor or third party promotional material. The personal details of finalists that qualify for the Z Manu World Championships will be shared with Quantum Events (the organisers of the Z Manu World Championships) for the purposes of facilitating entry into the event.
7. By registering for this event, I indicate that I have read carefully and understand this declaration, warrant, waiver and release, and having done so, I submit to it voluntarily.
