Before the Participant is allowed to access any coaching or sport science services, facilities or equipment, or undergo a fitness assessment this agreement must be signed

1. Fitness to Participate: I declare that I am medically and physically fit and able to participate in the Bounce session. The Bounce session will include tranpoline and 'dryland' equvilent activites as well as general fitness and conditioning activities (collectively known as “Diving Activities”). I will immediately notify Diving Queensland of any change to my medical condition, fitness or ability to participate. I understand and accept that the Diving Queensland will continue to rely upon this declaration as evidence of my fitness and ability to participate.

2. Warning: Diving Activities can be inherently dangerous. I acknowledge that I am exposed to certain risks during Diving Activities. I acknowledge that accidents can happen which may result in me being injured or even killed, or my property being damaged. I have voluntarily read and understood this warning and accept and assume the inherent risks in participating in the Diving Activities.

3. Medical Treatment: I consent to receiving any medical treatment that Diving Queensland or Bounce Tingalpa reasonably considers necessary or desirable for me during my participation in Diving Activities. I also agree to reimburse the relevant Organisation for any costs or expenses incurred in providing me with medical treatment.

4. Right to Use Image: I acknowledge and consent to photographs and videos being taken of me during my participation in the Bounce Session. I acknowledge and agree that such photographs and videos are owned by Diving Queensland and/or external media outlets and may be used for promotional or other purposes without my further consent being necessary. Further, I consent to Diving Queensland using my name, image, likeness and also my performance in the Diving Activities, at any time, by any form of media, to promote the Diving Activities.

5. Privacy: I understand that the information I have provided is necessary for the objects of the Diving Queensland. I acknowledge and agree that the information will only be used for the objects of the Diving Queensland. I understand that I will be able to access the information through Diving Queensland. I acknowledge that the Diving Queensland may also use my personal information for the purposes of providing me with promotional material from Diving Queensland sponsors or third parties. I may advise Diving Queensland if I do not wish to receive from the Diving Queensland, any sponsor or third party promotional material.
