This waiver must be agreed to before the Participant is allowed to access any facilities or equipment. If member is under the age of 18 years, parent/guardian consents on child behalf and the below "I" refers to particiapting member

1. Fitness to Participate: I declare that I am medically and physically fit and able to participate in the DQ sessions, including being competent to swim in deep water unassisted. The sessions will include activities in the pool and possibly other general fitness and conditioning activities (collectively known as “Diving Activities”). I will immediately notify Diving Queensland of any change to my medical condition, fitness or ability to participate. I understand and accept that the Diving Queensland will continue to rely upon this declaration as evidence of my fitness and ability to participate.
2. Warning: Diving Activities can be inherently dangerous. I acknowledge that I am exposed to certain risks during Diving Activities including but not limited to slipping, injury, water quality and contact and diving board contact. I acknowledge that accidents can happen which may result in me being injured or even killed, or my property being damaged. I have voluntarily read and understood this warning and accept and assume the inherent risks in participating in the Diving Activities.

3. Medical Treatment: I consent to receiving any medical treatment that Diving Australia, Diving Queensland or Venue reasonably considers necessary or desirable for me during my participation in Diving Activities. I also agree to reimburse the relevant Organisation for any costs or expenses incurred in providing me with medical treatment.
4. Privacy: I understand that the information I have provided is necessary for the objects of the Diving Queensland. I acknowledge that Diving Queensland may also use my personal information for the purposes of providing me with promotional material from Diving Queensland sponsors or third parties. I may advise Diving Queensland if I do not wish to receive from the Diving Queensland, any sponsor or third-party promotional material.

I have read and agree to the terms and conditions of the enrolment and understand that diving can be an inherently dangerous activity.
